Office Policies


Office Hours
Our office hours are Monday through Friday 8:30 AM to 5:00 PM

After Hours
Our office has a Physician and Certified Physician Assistant or Nurse Practitioner available 24 hours a day for our established patients. Call our main number (214) 370-3006 and our answering service will page the assistant on call. However if you are having a true emergency, call 911.

Appointments
You may schedule an appointment through our main switchboard at (214) 370-3006. If you need to reschedule an appointment or if you will be late for your appointment, we ask that you call as soon as possible so that we can call patients on our waiting list. Please bring your insurance card with you to facilitate your appointment processing.

Billing
If we are contracted with your insurance carrier, our office will file all insurance claims directly to your health insurance carrier. If you should have a change in insurance during your course of treatment, please communicate this to our office to avoid any unnecessary bills to you. All copays are due at the time services are rendered. Our office accepts Mastercard and Visa.

Disability Forms
If you have a disability form that needs to be completed by our office, please remember that it takes at least 7-10 days for processing and processing fees do apply. $10 per page with a maximum charge of $30. All completed forms can be mailed or picked up. Due to HIPAA regulations, our office will not fax any disability forms. 

Film Requests
Physicians are required to maintain medical records including x-rays for 7 years. Therefore, film is a part of you permanent medical record. If you would like to check out your neurodiagnositic studies such as MRI (s),CT (s) or Myelogram (s) you may do so by contacting our medical record department. You will be asked to sign a checkout form upon pickup. If you need your Plain X-Ray film (s)( x-rays taken at our office), you may purchase a copy at $8.00 a sheet unless you have been referred to another specialist by our office you may check out your Plain x-rays. 

Medical Record Requests
All medical record requests must be followed in writing by a formal written release. A copy of the North Texas SpineCare release is available to you via fax or mail. No hand written requests will be processed due to HIPAA regulations. You can fax or mail the request back to North Texas SpineCare for processing.

All requests for records will be processed within 15 business days. A processing fee of $25 up to the first 20 pages and $.15 per page thereafter plus postage (as mandated by Texas State Board of Medical Examiners) will apply unless the information is requested by a licensed Texas health care provider or a physician licensed by any state if requested for purposes of emergency or acute medical care. 

MRI (s) , CT (s), Myelogram (s), EMG/NCV (s), Plain X-Ray Films
If you are a new patient and have had any of the above studies performed regarding your area of complaint, we ask that you bring the film and report with you to your visit. This will provide greater detail regarding the medical decision making process. Without this information, could delay your appointment. 

Test Results

Upon scheduling your ordered test, you will be scheduled for a return appointment or placed on a call back list depending upon the request of the physician. If you are scheduled on a call back list, the physician ordering your test will call you within a maximum of 10 business days of your test. If you have not received a phone call from your physician regarding your test result, please do not hesitate to contact our office. 

Test Scheduling
At the end of your appointment, if were indicated for a test, our office staff will make the appointment for you. However on many occasions, test scheduling cannot be completed until the office notes have been transcribed. As a result, this could delay your test scheduling no greater than 72 hours. 

All appointments for testing scheduled by our office must be kept. If not, please contact us if you are unable to keep your appointment. If you no show for your scheduled test, you will be considered a non-compliant patient. 

Prescription Refills
To facilitate the processing of your prescription refills, please call you pharmacy for any refill requests. The pharmacy will in turn, call our office if they have any questions regarding your refill.

If you are requesting a new medication or one different from what you are taking, please call our office and ask for the Triage provider. To process any new prescriptions efficiently, have your pharmacy number available. 

If you are taking a narcotic prescription, you must be seen by a Physician, Physician assistant or Nurse Practioner every 3 months in order to obtain a refill.

If you are taking an anti-inflammatory prescription, you must be seen by a Physician, Physician Assistant or Nurse Practioner every 6 months in order to obtain a refill. 

Our office will process all prescription requests from business hours of 10:00 AM until 4:00 PM. Refill requests will not be processed over the weekend.
